Philosophy Of Care

Our service users are encouraged and given access to community facilities and services, Primary Health Care, education, employment, housing, welfare and recreational Services.

Service users participate fully or are represented in all forums, particularly where significant decisions are concerned. Service users right to choice and informed choice is respected.

Service users are valued as citizens with rights and responsibilities and we aim to:

  1. Support people to develop socially valued roles in the community
  2. Promote independence
  3. Overcome social stigma associated with people with disabilities by supporting individual
    users ensuring that the
  4. Service provision in no way reinforces these misconceptions
  5. Respect users privacy and confidentiality at all times.

Our service users are encouraged and given access to community facilities and services, Primary Health Care, education, employment, housing, welfare and recreational Services.

Our service aims to support individuals to develop the skills necessary to maximise their community presence, dignity and community participation. This will be achieved by designing, implementing and evaluating a programme of support, which is based on the unique needs of each individual.

Individual Service Planning

Each service user would have their individual care programme. An initial 3 months assessment programme is agreed upon and this is followed by a review 3 months later and afterwards reviews would be held six month.

The (I.P.P) Individual programme plan is based on the following key principles.

  1. Involvement of the service user, their advocate, carer and family, as well as professional staff in decision making and the achievement of objectives.
  2. A shared responsibility of all agencies to work effectively in partnership for the benefit of the service user.
  3. The service users progress throughout the care plan will be monitored and constantly supervised by the key worker and care manager.
  4. The home along with the Placing Authority and family members of service users do Person Centre Planning for service users.

This would be included in their care plan.
